insert merged field into free format address field

I have a survey where I need to pull in merged fields to a page that asks for the respondent's name and address.  I have no problem pulling in name, street, city, etc.  But the State in the free format question is a select from a drop down list.  Here is my code for city (which works) and state (where I do not have the correct code).  
The merged field I want to bring ins is called "City1"

Please help.

Try adding this JavaScript to the page:

$(document).ready(function(){
    $('#Contact_state').val('[% StateMerged %]');
})


Replace "StateMerged" with the name of the merged field for state.
