image does not show up when testing

I included pictures in one of my attributes,  when i click preview of the CBC questions, it was fine. but when it comes to the test page, the image does not show up.
here is my code

also another problem is if i adding picture code in attribute level, the 'checking error' shows this:  
Warning: CBC Exercise 'P2T3' - Attributes - The display text for some levels is long or contains special characters. Consider editing the text to add an additional "Internal Label" for easier reference during analysis and simulations.

I would recommend against adding any images with a direct file path.  In addition to test mode, this won't work when you go to upload your survey.

Instead, I would recommend using SSI Web / Lighthouse Studio's built-in tool for adding images to your survey.  In the text editor, click the button that looks like a camera.  The end result should be an image that looks something like this:

<img src="[%GraphicsPath()%]image1.png" alt="" border="0">

With the addition of "GraphicsPath," your image can now show up in previews, test survey, and the live survey.
